New Patients

How to Register

St David's Court Surgery welcomes registrations from patients who live within our practice Catchment Area.

Please check whether you live within our Catchment Area before completing your registration.

You can also search for local GP practices using  NHS 111 Wales - Services near you or telephone 01495 300742.

What You Will Need

All new patients are requested to provide:

  • Photographic identification (for example, a passport or driving licence)

  • Proof of address (for example, a utility bill, tenancy agreement or similar document)

Registration Forms

Registration forms can be obtained from Reception or downloaded from the documents below.

Completed forms, together with your proof of identity and proof of address, should be returned to Reception between 2:00pm and 4:00pm. Original documents must be brought for verification.

Once your registration has been processed and you are live on our clinical system, we will contact you to arrange a new patient health check with our Health Care Support Worker.

Registration Process

  1. Complete a GMS1 registration form and Patient Information Questionnaire.

  2. Provide proof of identity and proof of address.

  3. Register for the  Wales NHS App if you wish to access online services.

  4. Attend any health screening or medication review appointments requested by the practice.

It is important that all sections of the registration questionnaire are completed fully.

Patients who are receiving treatment for a chronic disease or who take regular repeat medication may be required to attend the Practice for a health review following registration.

Important Information

Failure to attend requested appointments following registration may result in delays to:

  • Medical treatment

  • Specialist referrals

  • Diagnostic tests

  • Repeat prescriptions and medication reviews
