Repeat Prescriptions
Repeat prescriptions are available by request, subject to regular medication reviews by the Doctor. Please allow at least 3 working days when ordering your medication.
- By Telephone - We do not accept medication requests over the telephone for patient safety reasons. Medication queries can be made by calling 029 2037 9510 after 11:00am, Monday to Friday.
- In Person - Repeat prescriptions can be requested using the white repeat slip attached to your previous prescription. Please tick the items required and either:

Post the request to the practice, or

Place it in the letterbox at the practice entrance.
- Alternatively, you may provide your name, address, date of birth and details of the medication required on a separate piece of paper.
- NHS Wales App - You can request your repeat medication using the NHS Wales App
- By Email - Please order your repeat medication by emailing SDC's Email. Please include: Full name, Date of birth and your nominated pharmacy. Please note this email address should not be used for urgent queries.
- Electronic Prescription Service (EPS) - Most prescriptions are now sent electronically to your nominated pharmacy using the Electronic Prescription Service (EPS). This means that you will usually not need to collect a paper prescription from the surgery. If you would like to nominate or change your chosen pharmacy, please contact the practice or speak to your pharmacy directly.
Medication Reviews - Patients receiving repeat medication may be asked to attend an appointment with a Doctor or our in-house Pharmacist at least once a year to review their medication. Any queries regarding medication changes may also be discussed with our in-house Pharmacist. Please ensure that you book any requested review appointments promptly to avoid unnecessary delays to future prescriptions.
