How to register

St David's Court Surgery and Salisbury Road Surgery welcome registrations from patients living or moving in to our catchment area.

All new patients are requested to provide photographic identification (such as passport, driving licence etc) and proof of address (such as utility bill, rental agreement etc).  

Registration forms can be obtained from Reception, or downloaded from the documents below.   We ask that these are completed and returned to Reception between 2pm-4pm (Does not include a Wednesday)  with your photocopies and original documents of your ID and proof of address. The documents will then will be checked. Once you are registered and live on our system we will ring and book a new patient appointment for a general check up with our Practice Nurse or Health Care Assistant

Registration Process:

1. Register at either surgery by presenting your medical card (FP4) or by completing GMS1 form which is available from reception at both surgeries.
2. Complete the patient information questionnaire
3. Register online for the NHS Wales App to enable you to make appointments on line and request any repeated medication. Login screen (nhs.wales)
4. Bring copies of Proof of address and proof of ID.  Minimal of 6 months’ date on passport.

It is extremely important that all questions are completed within the questionnaire and that you are made aware that you may be required to attend the Practice for health screening if you are under any health surveillance for a Chronic Disease or are on long term repeat medication.
